Toronto Transit Commission fined $200,000 in death of worker

TORONTO - The Toronto Transit Commission has been fined $200,000 for the death of a repair crew worker.
The TTC pleaded guilty Tuesday in court to an Occupational Health and Safety Act violation.
The accident occurred on the morning of April 23, 2007, when a crew was working in a subway tunnel between stations to remove asbestos.
The crew was using a system of work platforms on a flatbed car, but when they finished the job, one of the platforms wasn't retracted fully.
As the work car rolled down the track, the extended platform caught on a bundle of cables, hit the wall and rose out of its mounting holes.
It then hit the tunnel and flipped into the front of a car, killing the train operator.
